Thursday, February 10, 2022 COVID Vaccine Clinic for Kids Dose 1 signup sheet

Tri-Town YMCA in partnership with Jewel-Osco Pharmacy and School District 45 Villa Park will be hosting a COVID-19 Vaccination Clinic for Kids in the gymnasium at Schafer Elementary School. We are in need of volunteers for a variety to work of shifts at the clinic. All volunteers will be required to wear masks and be free of any communicable diseases, including COVID-19/COVID-19 symptoms. More information will be distributed to volunteers the day prior their volunteer shift. Please no volunteers under the age of 12. Volunteers should wear comfortable clothing and footwear. Some positions will be outdoors so be sure to dress for the weather. Volunteers must be vaccinated to participate in this opportunity. Proof of vaccination will be required prior to entry to the building for your shift. An email will be sent directly to you requesting for your proof of vaccination closer to the event date.
