Station 1- REGISTRATION - Welcome and register each KEEP Healthy event participant as they arrive.  Distribute folders in numerical order and send participants directly to Station 2 to complete survey.

Station 2- KEEP Healthy Survey - Have everyone sit to complete their Participant Survey- front and back.

Station 3- Height/Weight/BMI - Record height, weight and BMI on proper forms. Weight measurement is a requirement and cannot be skipped.

Station 4- Blood Pressure - NKF will provide automatic cuffs, manual cuffs and stethoscopes. You are welcome to bring your own.

Station 5- Urine Test - Assess a participants risk factors for kidney disease by collecting a urine collection cup in a paper bag. Place dipstick in machine and print out result. No previous medical experience needed for this station.

Station 6- Clinician Consultation - Physicians, NPs, RNs, PAs welcome. Familiarity with current chronic kidney disease guidelines and risk factors for kidney disease. Educate participants to ensure they gain a better understanding of their kidneys, risk factors for kidney disease, and ways to keep their kidneys healthy, as result of participating in the program. Provide general health information about the participant’s KEEP Healthy BMI, blood pressure measurement and ACR urine test result (if provided) without imparting specific medical advice or personal opinions. Respectful of patient confidentiality.

Station 7- Program Evaluation - Have participants complete program evaluation. Ask them if they need assistance completing the evaluation.

Station 8- NKF Resource Table - Volunteers to hand out educational materials to participants.
