Mateo’s Finding Nemo Party signup sheet

Thank you so much for teaming up with Box of Balloons! We have the opportunity to help a caregiver in need give their child a happy and memorable birthday celebration.

-Supplies Due: Tuesday, June 10
-Theme: Finding Nemo
-Child's name: Mateo (boy)
-Preferred Language: Spanish
-Allergies: no known
-Age: 2
-Size: 2T
-Colors: Blue and Orange
-Gift ideas: Any age appropriate toy with Nemo, Encanto, or Pocoyo. Kiddo likes to sing, so a musical toy that is age appropriate for a 2 year old.
-From the social worker: Single mother of 5 kids with disabilities, difficult to make ends meet.

Please have all items labeled with the child's name delivered to us at any drop-off location on or before Wednesday so the social worker can pick up the box on Friday. Cupcakes can be delivered on Wednesday or Thursday at the preferred drop-off location only.
